What is pincer impingement?

Pincer impingement involves excessive coverage of the femoral head by the acetabulum. With hip flexion motion, the neck of the femur bone “bumps” or impinges on the rim of the deep socket. This results in cartilage and labral damage.

What causes pincer impingement?

The Pincer form of the impingement is typically the results of retroversion, where the socket is pointed backwards a bit (rather than the usual situation where it is angled forwards), or where the socket is too deep. Very often, the Cam and Pincer forms exist together.

Does FAI need surgery?

Surgery for FAI can be performed using hip arthroscopy or open surgery. In hip arthroscopy, the hip is distracted and an arthroscope (a videocamera about the size of a pen) is used to look in the joint to see and treat damage that is found using two to five incisions that are about ¼ inch in size.

The most serious damage caused by pincer FAI occurs at the level of the acetabular cartilage at its junction with the labrum (Fig. 1). The cartilage lesion is localized in the area where the contact occurs, which is usually the anterosuperior portion of the acetabulum.

Pincer Impingement refers to an excessively deep acetabular socket, restraining normal hip range of motion. Pincer impingement in turn may be divided general into two types: global and focal.

What does FAI pain feel like?

FAI usually feels like a sharp pain deep in the groin area or in the front of the hip. It worsens with athletic activities, or with prolonged sitting. As symptoms progress, the muscles surrounding the hip will fatigue and become very sore.
